You might try reading the Calcium thread, and have you tried Imodium?Some people can get a bile imbalance (release more bile than they can recycle so even if the gall bladder was normal (and sometimes they don't check for "too much" as usually they are looking for "can't release any" it may be worth trying a Bile Binder like Questran. It is generally constipating anyway, and if there is some excess bile it binds that up.There is a New England Journal of Medicine review from back about 2001 that has doses for the Questran and Imodium if your doc wants to look it up.There is a recent study that showed some IBS-D types do well on a low carb diet. You would just need to make sure your portion sizes are high enough to maintain weight.FWIW starving as long as possible only makes the diarrhea and pain worse when you finally do eat. Much better to eat 300 calorie mini meals every 3 hours or so than not eat all day and then try to play catch up with dinner.
